Successful aging and safe transitions home requires a collaborative, interprofessional approach integrating healthcare, home accessibility, and social support. With nearly 90% of older adults wanting to age in place and over 70% needing long-term care, this session explores how aging-in-place experts, healthcare providers, and technology innovators can improve outcomes and reduce hospital readmissions. Family history, the second most popular hobby in America, fosters cognitive engagement and emotional well-being. Photo Reminiscence Therapy (pRT) helps combat loneliness, isolation, and depression, improving quality of life. This session introduces Family Matters, a National Genealogical Society, and Vivid-Pix partnership providing tools for memory sharing and connection.

Falls are the leading cause of injury-related deaths for adults 65+, emphasizing the need for universal design, home modifications, and fall prevention strategies that protect both older adults and caregivers. Attendees will explore remote monitoring, communication tools, and safety technology to improve care coordination and empower independence. By fostering thought leadership and collaboration, Certified Senior Advisors (CSAs) will emerge as key advocates for aging-in-place solutions. Through research, case studies, and actionable insights, attendees will gain tools to enhance patient experiences, prevent complications, and strengthen community networks for aging well.
